You must sign your Jericho REACH Participant Agreement (PA) to be part of Jericho REACH on January 1, 2023.
Please click HERE to download the PA. Current participants need only sign, scan and return the signature page (p.22) to jericho@jerichoreach.com. New physicians/practices will also need to fill out Exhibit A (p.23) and send both pages to the same email address.
